Hepatitis B is a viral infection that affects the liver and can cause both acute and chronic disease. The virus is transmitted through contact with infected blood or other body fluids. Laboratory testing plays a crucial role in diagnosing hepatitis B infection, monitoring disease progression, and determining the appropriate treatment approach. Understanding the various hepatitis B tests and what they indicate is essential for proper disease management.

The most common screening test for hepatitis B is the hepatitis B surface antigen test, also known as HBsAg. This test detects a protein on the surface of the hepatitis B virus. A positive result indicates current infection, either acute or chronic. The hepatitis B surface antibody test, or anti-HBs, detects antibodies that develop after successful recovery from hepatitis B infection or after vaccination. A positive anti-HBs result generally indicates immunity to the virus. The hepatitis B core antibody test, or anti-HBc, detects antibodies to the core protein of the virus and indicates past or current infection. This test comes in two forms: IgM anti-HBc, which suggests recent acute infection, and total anti-HBc, which can indicate either past or ongoing infection.

Additional tests help assess viral activity and liver function. The hepatitis B e-antigen test, or HBeAg, indicates active viral replication and high infectivity. When this antigen disappears and is replaced by its corresponding antibody, anti-HBe, it usually signals decreased viral replication. The hepatitis B viral load test, also called HBV DNA quantitative test, measures the amount of virus in the blood and helps monitor treatment effectiveness. Liver function tests, including measurements of liver enzymes such as ALT and AST, assess liver damage and inflammation. Elevated levels of these enzymes suggest liver injury.

For patients with chronic hepatitis B, regular monitoring through laboratory tests is important to track disease progression and guide treatment decisions. Testing frequency depends on individual circumstances, including the stage of infection, presence of liver damage, and whether the patient is receiving antiviral therapy. Healthcare providers use combinations of these tests to create a complete picture of hepatitis B infection status, immune response, viral activity, and liver health, enabling them to provide appropriate care and prevent complications.
