Hemoglobin A1c, commonly known as HbA1c or glycated hemoglobin, is a crucial blood test used to measure average blood sugar levels over the past two to three months. This test is essential for diagnosing diabetes and prediabetes, as well as monitoring how well diabetes is being managed. Unlike regular blood glucose tests that provide a snapshot of sugar levels at a single moment, HbA1c offers a broader picture of long-term glucose control by measuring the percentage of hemoglobin proteins in red blood cells that have glucose attached to them.

When glucose enters the bloodstream, it binds to hemoglobin in red blood cells through a process called glycation. Since red blood cells typically live for about three months, the HbA1c test reflects the average amount of glucose in the blood during that time period. The test result is expressed as a percentage: normal levels are generally below 5.7 percent, prediabetes is indicated by levels between 5.7 and 6.4 percent, and diabetes is diagnosed when levels are 6.5 percent or higher on two separate tests. For people already diagnosed with diabetes, the target HbA1c level is usually below 7 percent, though individual targets may vary based on age, health conditions, and treatment plans.

The HbA1c test offers several advantages over traditional glucose testing. It does not require fasting, can be performed at any time of day, and is less affected by short-term factors like stress, illness, or recent meals. This makes it more convenient for patients and provides a more stable measure of blood sugar control. Healthcare providers typically recommend this test at least twice a year for people with well-controlled diabetes and more frequently for those whose treatment has changed or who are not meeting their blood sugar goals.

However, certain conditions can affect the accuracy of HbA1c results. People with anemia, recent blood loss, blood transfusions, or certain hemoglobin variants may receive inaccurate readings. In such cases, alternative tests like fructosamine or continuous glucose monitoring may be recommended. Understanding HbA1c levels and their implications helps individuals with diabetes work with their healthcare team to adjust diet, exercise, and medication to achieve better blood sugar control and reduce the risk of diabetes-related complications such as heart disease, kidney damage, nerve problems, and vision loss.
