The hepatitis B surface antibody test, or anti-HBs, detects antibodies that develop either after successful vaccination or after recovery from an acute infection. A positive anti-HBs result generally indicates immunity to hepatitis B. The hepatitis B core antibody test, or anti-HBc, helps distinguish between acute and chronic infection and can identify past exposure to the virus.

Additional tests provide more detailed information about the infection status and viral activity. The hepatitis B e-antigen test, or HBeAg, indicates active viral replication and high infectivity when positive. The corresponding antibody test, anti-HBe, suggests lower viral replication when positive. Hepatitis B viral load testing measures the amount of virus in the blood and helps monitor treatment effectiveness. Liver function tests, including ALT and AST enzymes, assess liver damage and inflammation. These tests are often performed together to provide a complete picture of hepatitis B infection status.

Interpreting hepatitis B test results requires understanding the pattern of positive and negative markers. For example, a person with positive HBsAg and anti-HBc but negative anti-HBs likely has chronic hepatitis B infection. Someone with negative HBsAg, positive anti-HBs, and positive anti-HBc has recovered from past infection and developed immunity. A person with only positive anti-HBs has immunity from vaccination. Regular monitoring through laboratory testing is essential for people with chronic hepatitis B to assess disease activity, liver health, and the need for antiviral treatment.

Healthcare providers use these laboratory results along with clinical evaluation to determine the best course of action for each patient. People at higher risk for hepatitis B, including those with infected family members, healthcare workers, and individuals from areas where the disease is common, should discuss screening tests with their healthcare provider. Early detection through laboratory testing allows for timely intervention and can prevent serious liver complications.
